Full Time: 37.5 hours per week

An exciting opportunity has arisen for a Long Term Conditions Dietitian (Band 6) working within Oxleas NHS Foundation Trust. You will work closely with the Band 7 Diabetes Specialist Dietitian to provide an integrated dietetic service for people with long term conditions in Greenwich. You will be joining an existing long term conditions multi-disciplinary community team, to provide care to people living in Greenwich with Diabetes, Heart Failure and Chronic Obstructive Pulmonary Disease (COPD).

You will undertake a clinical role, having responsibility for a patient caseload and supporting services in the community setting and will be able to work autonomously and as part of a multi-disciplinary team. You will be involved in training, structured education, group sessions, individual and joint consultations in community settings e.g. clinics, home visits and residential care settings.

You will need to be experienced in the care of people with long term conditions and capable of making informed clinical decisions, in a variety of situations. A post graduate qualification in Nutritional Support/Diabetes/Obesity would be desirable. You should also have experience of delivering structured education programmes and training in delivery of Diabetes structure education courses would be desirable e.g. DESMOND/X-PERT training.

We are seeking a motivated, enthusiastic individual who will need to be committed to developing and improving dietetic services for people with long term conditions, ensuring patients receive a high quality service in the Greenwich locality. You will be part of a wider team of Dietitians who are committed to providing the best patient care possible within current resources.

You must be organised, computer literate and able to demonstrate excellent interpersonal skills with a commitment to continued personal development. Excellent communication skills will be required in order to educate, empower and support not only clients, but also the health care professionals and family members caring for them.
