Community Learning Disability Nurse - Transition, Band 6

Fixed Term (9 months)

Full Time: 37.50 hours per week

This is an exciting opportunity for a learning disability nurse with sufficient experience, to join our pioneering efforts to support young people with a learning disability in Bexley.

The post holder will be actively engaging with young people with learning disabilities and their support network, assisting those who are preparing for adulthood to access appropriate support from specialist services.
 
The post holder will be required to have a sufficient post qualifying RNLD experience. We are looking for someone passionate about
ensuring people with a learning disability have improved health outcomes as they leave children's health services, through transition to specialist services for adults with a learning disability. 
 
The post holder will work closely with colleagues in the children's and adult services, often working across organisational & borough boundaries as part of the assessment process.
 
 Further development opportunities will be available to grow as a leader/assessor/supervisor within a growing workforce.
